Initiation of Mediation / Conciliation
The request for Mediation/Conciliation will include two copies of: A short statement on the nature of the dispute; and The names, addresses, (email addresses, where available) and telephone numbers of all parties to the dispute, including similar details of any representative in the Mediation/Conciliation.

Mediation Agreement
Parties to a dispute may start Mediation/Conciliation by filing a written request for Mediation/Conciliation to the FCDR together with the registration fee in accordance with the Schedule of Fees of the FCDR. In the absence of a Mediation Agreement, one party may request FCDR to invite the other party or parties to join in a submission to Mediation/Conciliation.
